For Crop Insurance: Access program forms, endorsements and the Contract of Insurance.
For AgriStability: Individual producers (sole proprietors) must submit their income and expense (tax) information on the T1163 form to the Canada Revenue Agency, who shares this information with SCIC. Corporations and Co-operatives must submit all AgriStability forms to SCIC. An individual’s supplemental information (inventories, receivables, payables, deferrals and purchased inputs) must be submitted directly to SCIC.

This form will compile the supplementary information needed to process your AgriStability application and will accompany the income and expense information you provide for your operation. SCIC will use the information from these forms to identify the productive capacity of your operation.
2024 2023 2022Guardian Dog Rebate Application
Under the Wildlife Damage Compensation and Prevention Program, SCIC can provide $100 to help producers offset the cost of purchasing a livestock guardian dog. Producers requesting this funding should contact a customer service office to complete this form. Receipt of purchase for the dog and breeder information is required to be eligible for compensation. There is no limit on the number of livestock guardian dogs that may be compensated.
